Osteoarthritis of the Foot and Ankle

Sever’s Disease

Sever’s Disease causes pain at the attachment of the Achilles tendon to the heel bone in adolescents. Sadly it is often mistakenly brushed off as “growing pains”.

What is Sever’s Disease?

Sever’s disease (also known as calcaneal apophysitis) is a type of bony injury where the growth plate at the back of the heel bone becomes inflamed and causes pain. It is the most common cause of heel pain in children between the ages 8 - 14 years, particularly those who are active or participate in a lot of sports on a regular basis.

Sesamoiditis

Sesamoid bones are curious things. Rather than being connected to other bones forming a joint, they are connected to muscles by tendons. The largest sesamoid bone in the body is the kneecap, also known as the patella. Did you know there are also tiny sesamoid bones in your foot too? What are the symptoms of Sesamoiditis?

Diagnosing Sesamoiditis starts with taking a thorough history and examination of your foot. The forefoot - commonly referred to as the ball of the foot - is an extremely complex area, and is critical for normal and efficient movement.

There are many causes of Forefoot Pain which require careful judgement and assessment to confirm a diagnosis and form an appropriate treatment plan.

Common causes of Forefoot Pain:

• Overuse: Pain from overuse in athletes and those that participate in sports
• Toe deformities: Those who suffer from Hammer Toes or Bunions are more prone to forefoot conditions such as Metatarsalgia
• Arthritis: Osteoarthritis causes inflammation and pain of the joints, and commonly affects the big toe joint as well as the lessor toes

• Morton's neuroma: A painful condition that affects the nerves that run between the toes - the interdigital nerves.

• High arch feet: This foot type places excessive pressure on the ball of the foot, increasing the risk of forefoot injuries and pain.
• Loss of fatty padding: With age, the protective fat pad under the forefoot thins out increasing susceptibility forefoot pain and injuries.
• Poor footwear: Wearing tight, narrow or high-heeled shoes puts constant strain and pressure on the ball of the foot.
• Overweight: Excess body weight tends to put more pressure on the metatarsal bones and can cause pain.

Leg Pain

Our legs work hard to meet the demands of our day-to-day activities and commitments. This doesn't mean you should be in pain, though. Here are some common causes of Leg Pain:

Bones, Joints, Muscles

• Muscle cramps: sudden, tight, intense lower leg pain, usually happens when your muscles are tired or dehydrated
• Muscle strain: when a muscle gets pulled or stretched too far, the pain is intense and the area tender to touch
• Shin splints: when the muscles and tissue around your tibia (shin bone) become inflamed and painful, especially during walking, running or jumping
• Tendinitis: usually the Achille's tendon, although the tendon attachments of other muscles that have succumbed to overuse can also become inflamed
• Broken bones or sprains: Ankle sprains can be mild, only requiring minimal intervention, up to severe where you may have a broken bone, or avulsion fracture. Repetitive activities such as running, combined with poor footwear or function can lead to stress fractures - a small crack in the bone

Blood Vessels and Clots

• Blood clot: most deep vein clots happen in the lower leg or thigh, commonly caused by prolonged periods of inactivity like on a long flight or car ride, but also due to being overweight, smoking or certain medications
• Varicose veins: superficial twisted, dark blue or purple veins caused by weak valves and vein walls. Varicose veins may cause a dull ache, especially after standing
• Peripheral arterial disease: when the arteries in your legs become damaged and hardened, the arteries narrow or become blocked, reducing blood flow to the legs.

Nerves

• Narrowed spinal canal (stenosis) and sciatica: A common causes of stenosis is arthritis of the spine when the spaces in the spine become narrowed. Neural symptoms in the leg can also be caused by a herniated disc, which puts pressure on the nearby nerve roots leading to symptoms of sciatica.
• Diabetic neuropathy: Nerves can become damaged from prolonged periods of high blood sugar levels, causing pain in the legs along with numbness and less sensation in the feet

Ankle Pain

Ankle Pain refers to any type of pain or discomfort in the ankle joint and its surrounding tissues.

Warts

Plantar warts (plantar verrucae) are infections in the skin caused by the human papillomavirus (HPV). They can be painful due to their position on weight-bearing skin, and can cause embarrassment for some people due to their cosmetic appearance. Plantar warts are common, particularly in childhood, and without medical treatment, some wart infections can spread to other sites or members of the family.

When should I see a Podiatrist?

• The lesion is bleeding, painful
• You’ve tried treating the wart, but it persists, multiplies or recurs
